GHS Alumni Registration Form

Instructions:

1. Select your year of graduation. If your year isn't listed, contact me, so I can create a table in the database for that year.
2. First names and maiden names go into the "First Name" field, i.e., John & Susan (Miller).
3. Last names go into "Last Name" field.
4. City and State are optional.
5. Make sure your email address is valid, so your classmates can reach you.
6. Fill in your password two times - make sure they match. Keep it handy, this is your password when you need to Change Your Info.
